Ruptured splenic artery aneurysm in a third-trimester pregnant woman: a case report

Introduction: Splenic artery aneurysm is a rare condition but is associated with high maternal and fetal mortality when rupture occurs during pregnancy, particularly in the third trimester. Objective: To report a case of ruptured splenic artery aneurysm in a woman during the third trimester of pregnancy and discuss the diagnostic and therapeutic challenges involved. Methods: This is a case report of a woman at 31 weeks of pregnancy admitted with signs of hemorrhagic shock who initially underwent an emergency cesarean section, followed by imaging investigation and definitive surgical treatment. Results: The patient underwent splenectomy after identification of a ruptured saccular splenic artery aneurysm. Maternal survival was achieved; however, fetal death occurred. The postoperative course was favorable, with no late complications. Conclusion: Ruptured splenic artery aneurysm during pregnancy is a rare and frequently underdiagnosed emergency. Early recognition and prompt intervention are essential to reduce maternal and fetal mortality.
